Form Builder

Your clinic's forms, in your words, back as a signed PDF.

Admission forms, health questionnaires, consents and registration. Build them once, send them to a patient or a colleague, and file the answers without retyping a thing.

$75 a month plus GST, free for the first 14 days (up to 25 sends a day). Cheaper still beside another app.

Saved as you go

From blank page to filed answer.

The person filling it in sees one clean form. You see the whole history of it.

  1. Build the form, or start from ours

Write it in the editor, which shows the form exactly as the recipient will see it, or start from a Medervia template and make it yours.

  2. Send it

Choose a patient, someone new, or anyone in your address book. A form about a patient can go to a colleague with the patient's details filled in from your records, and a form prefilled by the clinic lets you fill in part of it before it goes.

  3. They answer at their own pace

Answers save as they go, on a phone or a computer. A long form becomes steps with progress, and they can come back to it.

  4. The answers come back as a PDF

Submitted, or accepted and signed. The recipient keeps a copy, and the clinic has the record and the PDF in its own document style.

Then try it yourself, step by step:

Build a form

Create a form, add questions and sections, show questions only when they apply, and publish it.

What Form Builder does

  • Every kind of question a clinic asks

Yes or no, choices, short and long answers, dates, times, numbers, email and phone, with headings, notes and highlighted text between them.

  • Questions that appear only when needed

Show a question or a whole section only for an earlier answer, so a patient never scrolls past what does not apply to them.

  • Paste in the form you already have

Paste your existing form as text or from a document and it becomes questions, dates, choices and sections, ready to tidy.

  • Or ask your own AI to draft it

Connect Claude or ChatGPT to Medervia and ask it for a form. It drafts; a person on your team reviews and publishes.

  • Versions that do not move under a patient

Publishing freezes a version. Edit the form tomorrow and every form already sent keeps the questions it was sent with.

  • New patients, without retyping

A form with a patient details block offers each new patient's details back to you as a draft, to accept into your contacts or match to someone you have.

  • Know what is waiting

See every form sent, who has opened it, who has answered, and which ones need chasing, from the dashboard or the list.

  • One library for the whole clinic

Owners and admins build and publish; everyone on the team can send, read the answers and print the PDF.

  • Signing is a deliberate act

The signer's name is filled in, and nothing is signed until they tick that they are signing. They can clear the name and type their own.

  • Prefilled by the clinic

Mark a form as prefilled by the clinic and sending it adds a Prefill step, where you fill in part of each recipient's form: the procedure, the site, the date. Those answers are marked as entered by you and the recipient can't change them. Nobody on the clinic's side signs; the recipient fills in the rest and signs as usual.

With Rooms, completed in the room

A form with parts for the practitioner or the practitioner's signature is completed in a room, so it is never emailed half finished.

  • Handed out in the room

Hand a form to a patient placed in one of your rooms. They complete it on the room's tablet, or on a colleague's device with them there.

  • Parts the practitioner fills in

Mark a question or a whole section as filled in by the practitioner. The patient sees it but can't change it, and the practitioner answers it in the room.

  • Signed by the practitioner too

A form that needs the patient's signature can also need the practitioner's, made on their own device in the room. The PDF carries both, side by side.

  • Filled in together, live

The practitioner and the patient work on the same form at once, each seeing the other's pointer and answers, and an answer the clinic changed says who entered it.

Part of your clinic, not a separate system.

  • Start from the patient

Every app can be started from a patient's record or from your contacts list, with several patients ticked at once where the app sends to many. Whatever an app produces for a patient is gathered on that patient's record.

  • Your records stay if you stop

Cancel an app and nothing it produced is lost. Its records and documents stay with your clinic and on each patient's record, and your patients keep their copies. Only sending new ones stops.
