A welcome from Southbank Specialists

Dear Grace,

Your first appointment is with Dr Amelia Shaw at our Southbank rooms on Tuesday 20 October at 10:30 am.

What to bring

  • Your Medicare card
  • Your referral letter

Before you come

  • New patient registrationFormDone
  • Bulk billing agreementTo sign

One email, one link. 1 of 2 done.

From template to a patient who arrives ready.

You write the letter once. Each patient gets their own pack, with their own details.

  1. Start from your template

Your letter in your clinic's own words, with the forms and the bulk billing agreement that go with it. Change anything for this send only.

  2. Choose the patients

One patient or several at once. Each of them gets a pack of their own, sent to their own address.

  3. Add each patient's details

The appointment, the practitioner and the location. The agreement's date of service follows the appointment, and you see what is missing before you can send.

  4. They open one link

One email from your clinic. The letter first, then each item to complete, on a phone or a computer, at their own pace.

Then try it yourself, step by step:

Send a welcome pack

Send a new patient one email with your letter and everything to complete before their first visit.

What Welcome Packs does

  • A letter that reads like yours

Headings, lists, bold and links, and highlighted words for each patient's name, appointment, practitioner and location, filled in when it is sent.

  • Everything that goes with it

Attach any published form from Form Builder and an Assignment of Benefit pre-agreement. A template keeps the letter and its items together.

  • Only the places that make sense

Choose the practitioner first, and only the locations where they hold a provider number are offered, so an agreement never names the wrong rooms.

  • See it before they do

The review shows exactly what each patient will receive, letter and all, with their own details filled in.

  • Easy on a phone

The patient reads the letter and works through each item wherever they are. Answers save as they go, and they can come back later.

  • Signed copies for both sides

A signed agreement or form comes back as a PDF in your document style. The patient keeps a copy, and so does your clinic.

  • Know where each pack is up to

Delivered, opened, partly complete or complete: see every pack's progress, item by item, and send it again or withdraw it if plans change.

  • Each item stays in its own app

A form in a pack is still a Form Builder send, and an agreement is still an Assignment of Benefit record, with the same history and documents.

Part of your clinic, not a separate system.

  • Start from the patient

Every app can be started from a patient's record or from your contacts list, with several patients ticked at once where the app sends to many. Whatever an app produces for a patient is gathered on that patient's record.

  • Your records stay if you stop

Cancel an app and nothing it produced is lost. Its records and documents stay with your clinic and on each patient's record, and your patients keep their copies. Only sending new ones stops.

Fewer forms on clipboards in the waiting room.

Start with the letter you already send new patients, attach the forms that go with it, and see who is ready before they arrive.
