Add a practitioner's part and signature to a form

Mark questions the practitioner fills in, ask for the practitioner's signature beside the patient's, and complete the form in a room.

An owner or admin edits and publishes forms. These two settings appear only when your clinic has Rooms.

Some forms need the practitioner as well as the patient: a consent the practitioner explains and countersigns, or a questionnaire with a clinical section. A form with either setting is completed in a room, with the practitioner there.

  1. Mark the practitioner's questions

Open the form in the Library and select a question or a section. In its settings, switch on Filled in by the practitioner. A section's switch covers every question in it.

The patient sees those questions but can't change them. The practitioner answers them in the room.

  2. Ask for the practitioner's signature

Open Form settings from the Actions menu. Under Signature, choose Signature required: the form ends with Accept or Decline, then tick Also signed by the practitioner, and save.

  3. Publish

Press Publish. The Library marks the form In the room only: it is not offered for sending by email or in a welcome pack, so a patient never receives it with the practitioner's parts left blank.

  4. Complete it in the room

Hand the form out to the patient in a room. The practitioner opens it on their own device with Open with patient, answers their questions, and signs under Your signature: tick I am signing this document, then press Sign.

The practitioner's name is filled in from the roster, with their provider number at that location if they have one. They need to be placed in the room, and to have signed in to Medervia in the last thirty minutes; if not, they are emailed a code first.

Good to know

  • The PDF has two signatures side by side, Patient and Practitioner, each saying it was signed in person and in which room.
  • If the patient declines, the form ends without the practitioner's signature.
  • Medervia's own templates have neither setting, because both need Rooms.
